How To Secure A T Handle?

How would you guys secure a loose T handle chuck wrench? I tried peening the knurled portion and it only hold for a while and then comes loose again. (I have a picture but I'm struggling with the picture in a thread software...)

"Preening" have I developed a new procedure... I need to find the "edit" button...
 
You could drill and tap the body for a set screw. Thas way if you ever need the extra leverage you can adjust.
 
Drill in from the end and tap for a grub screw. A small flat on the T will allow it to seat nicer.

I wondered about some form of adhesive, but this is one of those things that I find a really good solution after I have done something different that makes subsequent attempts difficult... I may try drilling it again. I must have been using a dull drill the first time, it sure seemed REALLY hard.

Remove the cross arm, drill a hole in the end, through the cross arm hole and about 1/2 in deep beyond that. Insert small spring and ball bearing, turn a groove in the center of the cross arm, Insert cross arm until ball bearing intersects groove. adjust dimensions to fit sourced materials.
 
Have you considered using two O-rings that are a tight fit on the cross arm, one each side of the body?

Just put a spot weld on each end of the handle so it can't come out.
