I have in the past been able to start the lathe with the belt engaged.
When starting the motor it started to click off the mains throwing sparks out the rear of the motor.
I replaced the Centafugul start switch thinking all would be good but still is similar. If I start with the belt disengaged it is about 90% good start.
Has anyone had this issue is it capacitor or maybe the motor??
 
Have you checked the bearings?
 
There would normally be a small amount of arcing when the start winding unloads. An undue amount of sparks may indicate a short in the start winding. A short may also explain why the motor is slow to start under load.
 
It's possible the start capacitor is failing (shorting internally), you could try a replacement cap but my hunch is you have a winding problem as RJ mentioned above
