how is this done?

like on the end of a ratchet wrench.
 
basically two ways, one drill the hole as required insert spring and ball the in a press deform the metal around the open end of the hole so it all holds together.
Two from the opposite side drill through with an undersized drill for the ball then drill again at the right size, but don't go right through, insert ball and spring drive a tight fitting tapered plug in and dress it off. More expensive, but a better job.
 
Drill/ream the hole. After installing the spring and ball, special punch is used to "upset" an annulus around the hole reducing the mouth of the hole enough to retain the ball. For onesie/twosie you could use a prick punch at numerous locations around the hole. You'll want to use some sort of clamp plate to depress/retain the ball if you use a prick punch. Relocate the clamp as you work your way around the hole.
 
Just a correction, because terminology can be important in technical fields: It's a "ball", not a "ball bearing." A ball bearing has multiple balls and spins.
