Bit by the Helix Monster

I've never had it happen- but then my machine uses Y style collets- are the R8 style prone to this problem? What about ER?
 
I learned this mistake too last year: had a bit of material to hog out of a 316SS bar stock; the EM pulled itself in a good 0.125"+. I chalk it up to an old, sloppy R8 collet and asking too much of the cutter (was a 3/4" HSS EM IIRC).

This is stock for 3 parts - hogging out material before sectioning. Look at that ramp! Ugh, didn't even notice until the cutter exited. This was the 2nd set I was doing. The first set I did using many much shallower passes with an indexable EM. The one below I tried going full depth with HSS EM. I believe the first full depth pass went fine, but the second pass with a stepover was what pulled in. I had wondered if the small stepover was part of the problem...?
