So over the weekend I finished and installed the replacement leadscrew and feed rod on my Bolton 13x40 project lathe. Apparently I got something wrong because the carriage stopped moving after engaging the feeds a few times.

Hopefully I can straighten it out well enough to at least make a replacement....

Aside from the bend is there other damage?
 
When you say again, did you heat it up and straighten it previously?
 
is that the hand wheel/ rack gear? What do you think caused stuff to bind up enough to bend it?
 
Thanks @Cooter Brown for the right way to fix this. I ordered a piece of 5/8 12L14 from McMaster Carr and proceeded to cut and fit it into the non-bent part of the gear.

Also filed the key that I think caused the binding in the first place.
