That is Part No 6 ... look at Part No 5; it is as I described with a conical feature that you grab to lift the Index Pin. You can also see
it in the assembly view.
 
OK. You are right. I don't think that you could actually make the knurl pattern as drawn. The diamonds would have to get larger as the diameter increased. But in any case, I suspect that you would have to custom-make the knurling rollers. I think that were I to need to make the parts. I would have to either skip the knurling or re-design the part. Or acquire a horizontal mill and indexer (which Atlas also sold).
