I took apart an old treadmill. The controls up top were damaged. The motor works fine. I read on line about using this controller to provide a signal to provide speed control. The frequency is adjustable but I don't know what to set it at. Any assistance in making this work would be appreciated.

that's a MC2100 controller by the looks of it. You'll need a PWM signal generator to control speed, either:

or there's a guy on homeshopmachinist called maxheadroom who makes little PWM generators for this controller for very reasonable money.

I have a couple of these and Arduino Unos on my bench to play with, pretty much next project in line :)

thanx Matt, I'll contact him.

yeah, I'd be real careful of using generic PWM pulse generators on these boards. I helped a guy build a 2x72 belt sander and he used a MC2100 board and a PWM pulse generator off Amazon. Fried the speed control part of the MC2100 so it ran full speed all the time.

Best off sticking with known quantities if you only have one or two of these boards.
