I got this in a box of Jacob's Chucks.

Looks like a tool holder from a turret lathe.

It is an adjustable tool holder for a turret lathe; if the turret holes are not concentric with the spindle or if tool shanks are not altogether straight, this flanked holder allows adjustment to put the end of the tool exactly on center.
