I recently bought a LeBlond regal 13 lathe and this was in the pile.

Looks like a 5C collet holder for a mill. Not sure how it would get used on a lathe but I am hardly an expert.
Figuring how it was used on a lathe was what puzzled me. A mill makes more sense. thanks
 
neat looking piece of kit!
....wooden knob, slotted indexing plate with spring-lock, "bent" angle lock handles.

Is there any manufacturer name on it?
Does it say "3A" on the collet:

Looks like a nice indexing tilt axis work head. But 3A collets are as antiquiated as the hand-carved hardwood handwheel that the fixture is festooned with. I would try to bore it out for 5C if I were serious about using it. Otherwise, it goes back on the shelf for another owner's lifetime to elapse, where it may see daylight again, only we'll probably all be in virtual reality suits to discuss it then.
 
I also suspect it's been modified. The support housing has a slide stop notch cut into it, and the index lever mounting bracket looks fabricated. I think someone converted it into a more versatile pawl wheel type of indexer from something else. So the wood handwheel is probably an add-on.

Critical IIRC, 3A collets top out at 1/2" when you can find them.

The plunger engages the spline for tightening and loosening.
