My favourite is not locking the wedge on the tool post after sorting out the height adjustment.
It must be my favourite because I seem to keep doing it.
Whats surprising is it cuts ok most times so I dont even notice till I look very close.

I forgot the ground on my TIG lots of times but the first time was the scariest. The high freq still gives you an arc but it won't melt butter! Of course my first thought is that the welder is history!
