Jonathan take a look at this diagram: does your switch look more like the "hockey stick" type or the "chopstick"/ 3-phase style?
To me it looks like the latter
Once you get the motor phased and we verify the switch type I can draw up a diagram to hook everything up--M

Chopstick. Took a while to figure that out. In these diagrams the handle is on top. I was backwards and pulling my hair then it got really clear.

Understood, it's possible the motor won't reach full speed without both windings in use. Anyhow it sounds like you are ready for this:

The switch was wired wonky. Much cleaner now and to your specifications above. Many Thanks!!! It’s running and I am on to looking for a modern replacement for the belt. I hope that I can return the favor some time on this forum.
