Head Tilting worm gear. Tighten the set screw in the slot on the shaft. Not too tight. It's a single not a double set screw.

Insert the Head Tilting worm gear shaft while holding the gear in the slot. Align the key in the shaft with the gear.

Make sure you can still rotate the shaft freely. If not back off the set screw until you can.
